What is a easy practical way to fix and extend an outdoor patio?

The patio was added onto with cement and the cement addition is cracking off at the border.

How long ago did you have this done? There is no continuity between the old and the new. Call the contractor who did the work, ask him to come out to see it, and what can you do. -

The house was built in 1950's. The additional cement was likely from the 1960's.
