I would replace the flapper. Your not getting a positive seal and water is passing past it. When you flush and the flapper closes it temporarily seals. Probably has gotten hard over time.
Also could be the flow valve itself also along with the flapper.

It might just be running and wasting water. You should adjust the flapper so that it does not tell the toilet that it is still empty. That is in the tank of the toilet and it is adjusted in most cases by positioning the float a bit lower and ensuring that the flapper closes after flushing.
