You need a concrete contractor to come out and assess the condition and advise what to do.
You will likely need to get the city involved as well, you may need a permit. Call a reputable firm to assess the situation, perhaps an engineer if it is really bad. Get several bids. My neighbor recently did that, but we live on a hill with lots of long term settling.
