I too have had to scale down my 5 acres of 'yard' and let a lot of it 'naturalize' -- I keep annuals in pots on my deck - just for color as my deck is deeply shaded I stick with Impatiens, Rex Begonias, etc., and then on the side of the house, within comfortable/safe walking distance I have potted Tomatoes -- I usually stick with the smaller varities - Sweet Sally ( salad tomato, similar to a Roma), and a couple of Cherry or Grape types that are very prolific, but small bush types.

I love gardening but am not able to kneel anymore. One great thing I found is raised garden beds. The larger home centers have them to purchase. They look gorgeous and are high enough to stand at, or sit my wheely cart next to, so I can still enjoy gardening my way.
You may have to go with small raised beds depending on your disability if you have the funds to have someone level your yard pathways are always good have them cemented so they are wheelchair friendly and plant along them
I have lived through this experience and have found ways to still garden, but I am not sure how disabled you are and it is such a personal question to ask a stranger. If you can give me a general idea maybe some of the ways I was taught by a Master Gardener could work for you too, thus allowing you the pleasure of being grounded in the dirt!! Good for you! Keep on gardening. Ask your family to put in some raised beds, they don't have to build them...go to the farm and garden store for stock feeding and watering containers, they are just the right height at about 2 ft. I have a larger garden in these containers than I had in the ground, so much easier to work with. Also, for soil I go to Home Depot and get the Kellogg brand (cheap) in large bags and used a few bags of potting mix on the top. Feed well and water. Good luck! Have a plan so the family does it your way!
