The type of sub-floor doesn't change the way you install the backer board. The backer board has markings to show you where to run screws.

Depends on how much give there is in the floor. If you stand between two floor joists and you can see that there is give in the floor, lay a second sub-floor that is not tongue and groove. If you plan to tile, you want the floor as rigid as possible or you will have issue with the tile and/or the grout later
