You might want to check for cracks in your fill tank.

Make sure the chain is loose on the flapper by a few links. Make sure the hose from the fill valve to the overfill tube is above the fill tube. Make sure the flush handle is loose and doesn't bind after a flush holding the flapper slightly off the seal. Make sure the float isn't binding after a flush. Next step would be to replace the fill valve. Sometimes sediment gets stuck inside the valve or the o rings fail.

Did you try tightening the chain, or loosening it? I hate toilet troubles
I have the same problem and my latest find seems to be doing the trick so far. Found a flapper at Home Depot, Fluidmaster Perfor Max , flush valve repair kit, fixes leaks and saves water. It has an adjust flush right in the flapper. Be sure to follow installation instructions.
Had the same problem. I bought a new toilet.
Scrape bottom of unit where flapper is not sealing completely. Had this problem and cleaning like this solved problem
