We added molding to the front of our old cabinets in the shape of a frame. It was really simple. Just cut the moldings to the size you need, then use carpenter's wood glue to attach them. Let dry and then paint as desired.

In my current home, no, but have in the past. Love it and adds so much character. They sell so many great embellishments now too. My suggestion would be to choose carefully, think about cleaning - nooks and crannies might require a toothbrush and a bit more maintenance.
