How well do these painted designs hold up? And do they need a sealer?

The paint used with the stencils will wear with traffic. unless it is sealed or baked into the tiles themselves.
Yes, use at least three coats of sealer. In high traffic areas reseal every two years. However, I little wear on stencils make them look set in instead of something that has just been added.
