Any good licensed building contractor can help you out. Is there a window in the bathroom? If so use it and keep it open while getting ready and if necessary, close before leaving the house. Chances are they can install better vents and a couple of fans.

A bathroom vent should be at least 100 cfm (cubic feet per minute) and most installations are only 50 cfm. Also, yes, make certain it is vented outside and no disconnects in the exhaust piping.
i would check that the vent is installed correctly (vents to the outside through the attic or wall) first. If it is vented properly, then it may not be strong enough and can be replaced with a better model. Also, is the crawl space insulated enough to stop the cold from coming up through the floor? It might help to ensure that the cold doesn't react with the warm moist air creating condensation.
How about installing a second vent?
