Cut the stems back right after the blooms are done. It helps them ready for the next season to bloom again. Leave some stem, I usually just cut just below the bloom area or no more than half way down the stem.
That depends on where you live. If you live . If you are in a colder climate wait for the first couple of frosts then you can cut it down to about 6 inches. If you live down south, you can cut it down once it blooms. Feed it and wait for fresh growth.
If your hydrangea is over grown you can cut it back quite a bit in the fall. A good rule of thumb is never to remove more that 50%. look for buds on the on the stems & make sure to leave some. If you want to cut it all the way back you can. Or you can prune & train it for next year.
