I would use it as a base and cover it with a piece of wood you like. This link will help. Then you can stain the legs to match the top.

You should sand it if the top finish has worn off, You want the top to be flat and smooth (no bumps or dips). Plus you new finish will need something to grip. A little extra work is worth a great finish.
Yes... you can put a glass or acrylic top on it... then do whatever you want under that....
You could put a wood filler in the bad spots...but that would require sanding... then you could paint the top any color you want... like dark gray, black, navy, or green.
Ceramic Tile it or Use Vinyl Tiles.. If you don't want to sand it, I guess you like the "Rustic look" In that case, slap any colour varnish on you like.......
