I have made them with polymer clay. You form it over a metal light switch cover (which can be reused). Bake and the pop it off the metal cover. You can make these look like anything you want.
You can decoupage with almost anything that will wrap around the cover...(I've printed a picture of my granite and used that to hide the plug). I decoupaged cut up old Thomas Kincaid desk calendar prints on inexpensive plastic covers, which go great with my yellow kitchen walls and we love the pops of color in an unexpected place.
