If you see mud tunnels, it is termites.
If the cracks are tiny you can put on a concrete resurfacer. (Home Depot and others). If they actually have a measurable width you need to replace it. Nothing you do will be a permanent fix. I addressed this myself two years ago and after paying for it there are no regrets. It was the right thing to do.
