We had a leak from the chimney and not sure how long we been here for five years we fixed it the 3rd year. Do we have to rip the plaster off or just patch it with plaster? Any thoughts thanks

I would love to add a mantle, but I think it would look funny on an off centered fireplace.
You may have a problem with your fireplace. It shouldn't get like that. I have it checked out before doing anything else.
Since the chimney is no longer leaking I'd sand and patch with spackle or wall board mud then sand again before painting.
Get a pro drywall guy and cover with 3/8 th inch dry wall. Or ask him to skim coat the whole wall. Plasters refer to it as a job number 5.
Don't forget to use a stain blocking primer such as Zinsser before painting.
