Tile OVER tile? I am not an expert, but this sounds like a disaster waiting to happen.
Are you wanting to tile over an existing floor tile or an existing wall tile? what type of tile are you wanting to cover? plastic? ceramic?

If you want a different look and a stone looks good for the area check out airstone.com But as far as tile over tile and adding more grout? If on a wall that is a lot of weight. If you want to do your floor go to a local flooring store there is a product that is thinner and will go over existing tile, but the key thing is it will change the transition areas to a room if you are not careful.
Another reason to go to store is because I recently went and the sale was so huge that it was cheaper to hire them then to take my husbands time and free him from the aches and pains!
Love Today's Homeowner! That is a lot of work! Sometimes wonder if they look at ramifications. It would also raise the floor obstructing the door opening. Raise the floor above the toilet flange requiring a flange retrofit. Transition between the bath floor and entrance would be higher and a possible trip hazard. Best to remove old flooring. Tiling over wall tiles can be a weight problem and gravity could delaminate the tiles off the walls.
