I've painted over brick fireplaces before, but this stone is rough. The top of the hearth looks like smooth marble, but the bottom is the same rough stone.

Yes you can! But you know what would be fab? That back splash tile! I think that would look cool there! Best of whatever you do!!
I did mine two years ago. Buffed brass with fine steel wool and used rustoleum high heat paint. Looks great, have used fireplace many times and no problem.
