Yes - please clean it every fall. It can harbor mites.
Bird houses should be cleaned to help prevent spread of disease and parasites. Any time when no nesting is going on would work but best might be late winter (or fall if winters are bad). Get it done earlier than you think they might be nesting as they "check out" the neighborhood when choosing a nest and you would not want to disturb them.
I agree with Sal85902.
Clean it out when the babies have left the nest so that the adults can start building a new one. The old nest will be very dirty, and possibly have parasites. Clean the box with soapy water and rinse well. Good luck!
PS: Sometimes birds will use a bird house twice in one season.
