I'm surprised vinegar didn't work. Did you use pure vinegar? I usually use regular white vinegar, generic, for cleaning but either white or cider vinegar will work.
I have not tried it myself but have read that denture cleaning tablets will clean stains from dishes. Read the label before trying but I think it would be safe for your cruett.

Most likely the glass is permanently etched by phosphates in combination with "soft" water. This surface erosion or pitting is harder to remove than hardened calcium deposits from evaporation because the surface of the glass is damaged. This can sometimes be corrected if the vase is "tumbled" by a professional glass repair service, but that is very expensive and I wouldn't recommend it for anything other than the finest crystal. Etching can be prevented by changing the water every day, but there isn't much that can be done after it happens.
You may try muractic acid. It is a pool supply. Read the warning labels very carefully before you proceed.
Or you could try a soak in Peroxide. It's 99 cents a bottle at WalMart. If it doesn't work, you aren't out much.
Try denture cleaner something like polident. James could be right that the glass is etched. This happens in automatic dishwashers because the soaps are so harsh
If the glass etched nothing will work.
