Hi Anne. At the bottom of the door where it sits in the floor bracket, should be a nut that you can turn to raise or lower the height of the door to keep it sitting in the tracks. Constantly opening and closing the door usually moves the nut and the door falls from the rail.
You would need to create a stop at the end of the rail to stop the door from being able to slide down that far.
Install a new track and new wheels.

OK, then, try to adjust the wheels to raise the doors off the bottom track. The doors may be riding on the bottom track and in turn it raises the doors just enough so the wheels on the top track are not sitting on their track and slip out. That's when they come off the track. The top wheel should alway carry the weight of the doors, the bottom tracks are there only for guidance.
Try that and hopefully it works.
