Square rain shower heads come in many different mounting styles and attachments that vary with the brand and price.(4 different examples shown below).
Most likely it will be easier to simply replace the entire fixed head unit, rather than adding an extension, but until we can see what you have now it is difficult to answer your question.
Please post a picture so that we can see what you are starting with.
You can get a shower neck extension. First remove the shower head, clean threads and then wrap with fresh Teflon tape wrapping in a counter clockwise direction. Then thread on the extension. Repeat the process to reattach the head.
