The ADA (American disabilities act) has instructions on the specific measurements required. It talks about the angle, etc, so you have a guideline on how to build.

A lot of local builders' associations do ramp-a-thons (where they build ramps for people's homes) as a service project. You might be able to get them out to do out. My neighbors had them come about a year ago.
