Depending on age of home, check piping beneath sub floor, as well. Cast iron pipes in older homes can also develop pin point leaks, leading to much bigger problems when they eventually collapse...

You will need a new toilet tank gasket and toilet wax ring. Remove the toilet tank off the toilet and set aside. Remove the toilet and set aside. Use a blow dryer to warm up the tiles and they should peel off. Cut the damage wood out close to any joists. Sister 2X4's to the joists. Screw new wood the the 2X4's. Glue the tiles back on. You may need to use tile adhesive. Replace the toilet with the new wax ring. The install the tank with the new gasket. Connect the water line, turn the water on and look for any tank leaks. flush the toilet and look for any toilet leaks. No leaks your done.
