Wash with soapy water, let dry outside then sand it restain and varnish. I have found this is the only way to get rid of that disgusting cigarette odor.
Have you tried applying a paste of baking soda & water? You will probably have to do it several times. TSP - trisodium phosphate - the cleaner you use before painting might also be helpful.

GET A SUPER LARGE BLACK PLASTIC LAWN BAG AND PLACE THE TABLE INTO IT, THEN ROLL UP NEWS PAPER WADS AND PLACE INTO THE BAG ,[THAT WOULD BE A LOT OF NEWSPAPER BALLS ] SEAL UP THE BAG AND LET IT SIT FOR A WEEK. THEN OPEN UP AND YOU WILL BE SURPRISED AT THE REDUCTION OF THE ODOR.
