If it's a concrete one, a good scraping of the old paint, perhaps some filling with new concrete, then an environmentally safe paint. There are many on the market today.

You will have to put a waterproof sealer on it. Remove it from the outside, & make sure it completely dries. NOTE: Tape off all entry's & exits of water fountain, so flakes, paint, & sealer don't get into them. Remove all of the old paint. If you have an air compressor, blow all loose flakes off, & spray with desired color. 3 coats is good. After this, use something like Helmsmann outdoor sealer, to completely coat entire surface. Use 3, or 4 coats. It must dry for a toatl of 4 days, & then another day or 2, for all the toxins to be released. Put your water fountain back.

what i would do & have done , remove all of old flaking material you can, wipe the surface with acetone to clean it further, then happy marine varnish , [ buy at Home Depot or Lowes ] its a fiberglass resin you add a catalist to set it up, if you want a specific color you can add that before adding the catilist mix throughly apply by brush, roller or spray if spraying , Don't breath the fumes & clean your spray gun throughly , apply 2 coats & your good to go drying time [ depending on the temp & humidity 24 hrs.
