Has this decor effect been put onto the drywall afterwards, or is it integrated? Many homes in the southwest have this style, which resembles more of a textured plaster. If it is the latter, just sanding will not be sufficient. The only way that a smooth finish can be achieved is to remove the wallboard, install new unpainted drywall, then paint new.
I would. You want a smooth finish prior to painting. I would also use a primer before using paint for an even better painted surface.
