Worry about the lead only if you are sanding the painted area or the paint is peeling - or if you have a child or pet that will chew on the woodwork.
Paintis not made with lead and hasn't been for what? Decades. Unless you are scraping off very old paint in a very old house, it will not be an issue.
The best thing is to paint a primer over the old paint and then put on your finish coat. This way you will not disturb the lead, if any, in the old paint.

I would wipe down the area and put a primer on it before you paint it. Sanding it first will allow the particles to get in the air. If you really need to sand down an area make sure you wear a mask. They sell primers to cover oil base paint. You can't buy oil based paint anymore but, the rule usually is once you apply oil you can put water based paint on top. The paint sells people are really helpful too.
