It sounds like there was dirt and/or moisture on the wall before painting.

The wall may not have been prepped right or the wrong paints were used for what was already on the wall.
Another cause is moisture in the walls from a leak. Check the wall surface behind the peeling paint for crumbling, swelling, or discoloration. If it is moisture in the walls, you need to fix the leak and repair damage before any durable painting can be done.
