Wil, first you need to find the source of the rot. You obviously have or had water coming through to the structural part of the bathroom at some point. Once you are certain that this problem has been cured, you can replace the subfloor. Chances are, you will need to pull the toilet and possibly other fixtures depending on the extent of the damage. Replace the it with good 3/4" OSB or plywood. After that, you can replace flooring and fixtures.
You have a water leak somewhere. Need to pull out old subfloor and remove the fixtures (tub, shower, toilet, vanity). Then start over. Time for a total redo.
Are you talking sub floor if so you will need to replace then add new flooring.
