What's best product & method to repair outside of concrete foundation?

I have areas on my outside concrete foundation which is breaking away from foundation. I would like to know the best way to patch these areas with a product that will adhere to the existing surface and method or procedure to prepare the damage surface. In your opinion does this a DIY type job.
