If your vegetables/flowers seem to still be growing well in there then I wouldn't worry about it. You seem to be following the right moves.
I would say no. If you are adding a bag or two of fresh soil every year, you're putting back the nutrients that were depleted by the plants last year.
You definately need to amend the soil! The nutrients can be depleted after 6 years. Add compost, manure and peat moss.
No just amend the soil.
I turn over mine over and add compost to it, especially my eggs, and bananas peels, I wouldn't, just add natural things to help detour bugs etc
If you are always adding soil improvements I don't think you need to replace it all. The only exception would be if your previous plants in the planter had a disease or an infestation. Then I would clean it out and start over. I have a couple of Earth boxes that have the same soil for four years. The nutrients will drain out over time, but if you are feeding and refreshing as you are doing I would say you're good to go.
