Hello. Have you tried CLR? I would try a smaller area first and then rinse to make sure it doesn't wreck the finish. Coca Cola also helps disslove rust. Good Luck!
If it is on the surface, try

If the rust stain goes all the way to the cast iron, that is, the porcelain finish is corrupted, you should still remove the rust and sand the surface well so that you can reglaze the sink.
One product for doing so: https://www.amazon.com/dp/B000PTSBKW/ref=asc_df_B000PTSBKW5212583/?tag=hyprod-20&creative=394997&creativeASIN=B000PTSBKW&linkCode=df0&hvadid=167155426463&hvpos=1o1&hvnetw=g&hvrand=10438459489684722318&hvpone=&hvptwo=&hvqmt=&hvdev=c&hvdvcmdl=&hvlocint=&hvlocphy=9021554&hvtargid=pla-309379953833
I had a sink that looked like that. We tried everything including ZAP which is supposed to work. It did remove a little (*very little) but never did remove a whole lot. We ended up taking it out and bought a new one.
If it seems to go all the way down then you will need to reglaze it.
Ok thanks
I HAVE HAD GREAT SUCESS WITH PLAIN OLD KETCHUP.

Bar tenders secret works well on my porcelain sinks and tubs.
