I don't know about the primer, but did you use a Concrete Paint? were the sealer and primer concrete products? was the concrete throughly dry before you started each coat. Power wash it to remove peeling paint, then just paint with concrete paint.
Another option would be to stain the concrete. Would you consider that?
By chance did you pressure wash your porch first before priming it? If you had any dirt on your porch the primer nor paint will stick very well and will crack and peel. I would pressure wash it and let it dry well and then prime and paint
You might also be getting moisture up from the ground through the concrete. If so, the only thing that can be done is to remove the paint.
