Anything can be painted. If you really like the sink, have it professionally painted and cured. Otherwise, replace.

We watch the Home and Garden channel a lot. I have seen a couple episodes where the cast iron claw-foot bath tubs are re-glazed and come back looking like new. Maybe you should consider having your sink re-glazed too.
