Kind of looks like an old letter opener. There used all kinds of things for handles back in the day.
Don't know, but I would assume it is some type old fashioned carpenter's tool. Could it be an awl? Just a wild guess.

It's a reamer. Used to widen holes in metal or wood. Looks like it had a wooden handle at one time that may have broke or came loose and replaced with the antler.

I have one just like it. Mine is a set. A carving knife, a serving fork, and the piece just like yours is a knife sharpener. I inherited mine also. And it does belong in the kitchen with all of us ladies. Oh and to be clear the antler handle is the original handle.
Not a knife sharpener with only six sides. They did make knife sharpeners with antler horn handles. But the steel part has a multitude of fine ridges for sharpening knives.

At Grandma house.yes she had one.part of a carving set. Was very neat the one she kept.
I would guess, it is the dagger from a walking stick, for protection.

II'll go with William, if it's 6 sided its for knife sharpening. My mother had one and a couple of large carving knifes that also matched.

It is made by Sheffield. It is Sa knife sharpener. It is a horn handle. It was also user to insert into a roast or bird to aid in picking out of over with long pronged fork.

It is a REAMER !!!! A letter opener has one or two flat sides, that it can slide and cut the paper. A knife sharpener is a round rod with a multitude of thin lines going down from the handle to the tip.
