I'm afraid this requires an expert! It would be very hard to make a new "top" layer that wuld actually stick to what is left of the sidewalk and not crumble off too. Good luck!

If it's a sealer or paint that's peeling up, you could powerwash it off and then put a new coat over top. Maybe self-leveling cement? If however it's the concrete itself deteriorating, you're not going to be able to save it. Maybe look into pavers?
To address the problem after it has occurred requires covering the entire affected area with a polymer-modified cementitious overlay in a color matching the existing driveway. Once the overlay cures, apply a waterproofing sealer to prevent the problem from reoccurring.
