Sounds like all you need is a new flapper inside the tank.
That sounds like you have a pipe problem, not a toilet problem. But yes, a toilet installation is diy. Heavy messy work to remove it though. You may need a helper.
It can be if you can manage the weight of lifting it or have help. I have done many times. Hardest part is lifting high to lower on closet bolts on each side. The rest is assembly, a piece of cake. Installing new or replacing existing very similar. Removing previous wax ring is messy. They have waxless choices now, which I used the last time there was a leak. How to link attached. Good luck

If you are saying it is filling up near the top of the bowl, then you have a stopped up pipe. You may need to call a plumber!
Sounds like you just need new innards in the tank as you have a leak, not needing to replace the whole toilet. That will do it if the flapper is not seated correctly.
