Take a sandwich bag, fill it with vinegar, put it over the shower head and use rubber bands to keep in place for a few hours. Then take it off and use an old stiff tooth brush to clean the shower head off.
If it won't come off, thoroughly soak a large soft cloth in distilled white vinegar. Wrap it around the shower head and tie it so the cloth touches where the water comes out. Leave it overnight. The next day, your water should run more freely. You may need a better wrench to loosen the shower head, or the threads may be "stripped".
They all come off. It may be just screwed on too tight. You can fill a food storage bag with vinegar and wrap it around the shower head with a twist tie. Let it sit until you see no more bubbles from the deposits being broken down.
Take a gallon size zip lock style bag. Fill with a cup or so of either white vinegar or CLR and place shower head in bag so that the vinegar or CLR covers all the holes in the shower head. Tie up with rubber bands or string. Let sit at least a half hour. Remove and turn water on. If really cruddy from mineral deposits, take an old tooth brush to it.

put white vinegar in a plastic bag and tie it on the shower head... let it set over nite and scrub with a toothbrush... works wonders.
put lime removal product in a plastic bag and tie it over the head.
