Inside your house Starting at the mantel - , clean with TSP, a scrub brush. Wear gloves and goggles and start form the top. For the inside of the fireplace, hire a professional.
I would add to wear a mask so you don't breath in the solution.

If you are talking about the interior of the fireplace, get a chimney sweep to clean and inspect for a safe fireplace and to prevent fireplace failure.
