What do you want to replace it with? You will need to remove the tile and cement board underneath. I would recommend getting a floor scrape. We usually break the tile with a hammer.

What kind of tile?
What type of flooring do you want to replace the tile? If vinyl plank flooring is among your choices, there may be no need to remove the tile. A high quality underlayment and good vinyl plank will allow you to put the new flooring on top of the tile. We've done this in three bathrooms. No sledgehammer required.
Remove the tiles, scrape off the adhesive. Remove the toilet. For ceramic tile you would need to put down cement board over the wood then tile. For vinyl plank flooring lay down wood underlayment then the vinyl tiles. Replace the wax ring and install the toilet.
