You'll need a little submersible pump and then build around it with whatever parts you'd like to use. A big bowl or pan of some sort, or rubber lined ditch will catch the water.

Indoor or outdoor??? The basics are a waterproof container, a submersible pump, tubing to bring the water from the pump to the 'waterfall' and waterproof decor of your choice. Oh, and water. If it is outside remember it needs to keep running so you don't breed mosquitoes - and if it's small (less than 100 gallons), dismantle it before your first freeze. If it is indoors, you should probably aim for a very small pump that has an adjustable flow so that it just bubbles the water out and onto the decorative medium.
