Use epoxy paint, mine has held up for 5 years and counting. Just follow prep instructions and won't have problems. Most hardware or big box stores sell kits.
I painted over a 1930s pink bathroom, too. Use the special tile paint. The kind I used was two-parts. The first part was a color while the second part added a shine so that the painted version looked more like ceramic tile. One of those parts (I think the shine) stank to high heaven. I had to leave the house for a couple of hours to let the stink die down. It lasted for the four years that I needed it. I saved up for a gut rehab.
What do you mean by "horrible underneath"? If there is mold under the tile you have a leak problem that needs to be fixed first. If there is a leak and it is covered over you may end up with studs that are rotted and would need to be replaced. It may encompass taking out all of the tile, putting up new backerboard (made for shower and tub walls) then retile. If a leak is causing your problem then you will need to find the leak and fix it before doing anything else, otherwise you are wasting your time and money.

Below is a video that shows the steps to painting ceramic tile. He is doing a kitchen, but the same method works for the bathroom. The only thing is that don't worry about the brand of paint. I would go to your local hardware store, Lowes or Home Depot for a good primer like Kilz. In addition, I agree with the others that an epoxy paint will hold up better, however, I did look at about 10 videos before choosing this one and one of them use a very high quality latex enamel paint for the top coat (2 coats of paint). She also said it was scrubable but others said you can't scrub it. Use your judgment when you talk to the people at the stores for what type of paint to use. I'd get ideas from more than one store and I'd also ask for the paint specialist in that store. I hope this helps. Wishing you the best.
