How to dress up my builder grade "clamshell" door molding ?
I would like to do an add on rather than remove the molding, any ideas?

Easy peasy. Get a narrow edge or chair rail molding and place it on the wall and inch or two away from and parallel to the existing molding around the door. Paint the new molding, the old molding and the wall in between all the same color and it will look like you spent a fortune. You can use a trim with a profile like this one:
I agree with the previous by adding a little it can make it look like a lot.
How about paint it then paint it with diffrent colors that go with the room like if there is a leave on it paint it green
