Use a bleach-based spray cleaner. Spray thoroughly, and let it set a few minutes, then use rubber gloves and cleaning cloth to wipe down your garden hose. If possible, leave it spread out flat to dry.
I used a clorox wipe on my white vinyl railings off the deck, worked well, but I think the Mr. Clean sponge worked better.

Start with bleach to clean off the mildew as described above. Wipe or spray down the hose with vinegar and leave it to dry without rinsing. Best antibacterial and antifungal treatment around for being such a cheap liquid. It is used on reusable medical supplies in the home to disinfect them for the next use.
