Either 2 x pieces of fabric 1"bigger all round than you want the finished cushion to be. A Zip or Velcro just short of the finished cushion size. Stitch Zip or Velcro in place . Then Sew the other sides (all from wrong size). Turn out and insert cushion filler.

If you don't sew, there are fabric glues you can use to put it together as Johnavallance82 suggested. If you use Velcro, don't use the pre-glued type, use the fabric glue, the adhesive on the Velcro won't hold real well.

Even at their most difficult, decorative pillows are easy to make. I like to put zippers in mine and also an edging. Either a welting or fringe or something like that. First I measure the inner pillow. If the zipper is put in the seam, the pillow can be reversible. I usually start with the zipper. Make a seam in what will be the bottom of the cover, just baste this seam, press it open. Lay the zipper face down on the seam. Stitch it in place with a regular stitch. Rip basting stitches open and remove them. Now you have an opening for your cover. Sew around the other three edges. Clip the corners, Turn inside out and insert the pillow. The pillows on the bed below have self welting and zippers. Welting can be a little confusing but once you get going right it is very easy to do. Then the pillows always have nice edges. I have made literally dozens of pillow covers.
