Also try Calgon. Hard to find powdered anymore but you can find at least in PA the liquid. It MIGHT help. I also use it in my toilet tank to keep the water soft and hopefully not leave anything on our new low flow tank.
Dilute the vinegar before use. There are other commercial products that work. You can try CLR. I used to use that to help remove scale inside my whole-house humidifier, and it also will dissolve scale on shower heads.
Best bets for the future: Don't let water sit on the counter of fixtures. I wipe mine down with my hand towel and I squeegee my shower glass after each shower. A water softener can also help prevent hard water damage.
NO vinegar on granite, it will ruin the countertop.

For faucets, you can use vinegar, apply to a cotton or microfiber cloth and wipe clean.
