To determine if the bush is dead scrape the bark of the bush to see if there is any green,if not chances are no.
roses are semi-evergreen. They should still have SOME leaves on them. If your bush has no leaves at all, it's probably dead dead. In that case, I'd do what Janet said to do, just to make sure.
Was your rose bush thriving and growing in the warmer weather?
They do go dormant in winter. Depending upon where you live, they do need to be cut back to the main canes. Where I live, I do this in February, as my roses are still bloomimg. Check with your local nursery and see what they suggest.
I live in northern Virginia. Our knock-out roses were just cut back last week. They will rebloom again next year.
I live in Missouri too...St. Louis area. Japanese beetles are a pain in the rear...but they don't kill the bush. They will eat all the leaves though.
Really, I think your bush will come back in the spring. It's just going to look pitiful until then. Maybe get some insecticidal soap to fight the bugs with.

I usually prune back in late February, early March.
I have cut most of my knockout roses back-still have a few to do- If you like you can root new ones from your clippings- I have rooted many and replanted or gave away .
