I usually harvest when the leaves are about 4 inches long. Gently tear or cut the outer leaves close to the bottom, but don't cut through to the crown or you might damage the plant and it will die.

I just carefully tear or snip off with a pair of kitchen scissors. Leave the plant itself in tact and it will continue to produce all summer long.
Don't pull them out, cut or gentlly rip off the Lettuce leaf.
Cut them but do not cut back all the way to the dirt and they will produce all summer
