looks like you need a flat surface to make the tile stick to something
Durock, or any other brand concrete backerboard for tile would work. Just extend the front piece on out to meet the side piece so you have a nice, square "box" to tile on. Do take the time to use the special webbed tape and thinset to cover any seams, including those corners. This will give you a more stable surface to work on so you don't get cracks in your tiles later on.

If you are going to cover the whole thing, use cement board.

This is great advice as far as it goes. To truly be stable, it needs a wood frame screwed together to attach the concrete board to, then the finishing details mentioned above. Good luck!
I'm just sharing my opinion, in my last house and I was trying to see if I had any pictures of how we transformed this same style fireplace that I hated too, but then there was no site like this and no pictures. What I did was had a handy man place a huge mantel around the block as I called it with the same thickness and I repainted the brick first then the mantel got installed and I loved it. I'm guessing from your photo that your living room walls are that tint of light mustard yellow color then I think aa dark grey or classy black paint? I hope your project makes a focal point in your home😎
