Does anyone know what type of cover goes on this outlet?

that looks older. You may need to look at a vintage restoration hardware place or remove and replace the outlet.
That is right, it looks old. But if you don’t replace the outlet, Lowes has oversized covers for this problem. They may have one that will work.
You can buy outlet covers. They come in two sizes, also in white or beige. They screw on.
Did you take the photo to home maintenance stores? Seems to me that would be the easiest way to find A cover that will work.
There are plates available at home improvement stores to cover such outlets. Be sure to buy the one to fit yours, there are two or four outlet plates. Yours looks to be a two hole outlet. They are easily applied with screws. See photo
I would be inclined to replace the entire outlet with a new one. The cost is little and and the process is easily conquered after watching a couple of YouTube videos.

Take your photo to an electrical supply house where electricians buy their material. They will be able help you and will probably know an electrician who never throws anything away that will have one.
This is a quite old plug - notice that there's no larger section for the new wiring. It's possible your new wiring may not fit in this. Test, and if that's true, you should switch it out. If you can use today's wiring, Wanda is spot on about going to an electrical supply house. Good luck.
It's an old outlet. You can try Home Depot, Lowes, or Menard's. I think a single gang rocker switch cover may work.

It looks like a mobile home outlet and the covers just snap in place.
Try a standard Decor Receptacle Plate. The inside openings will be square and fit the top and bottom (the middle will be covered up) but the screw holes to hold the plate on will match those on your outlet.
I would upgrade the outlet for safety's sake and solve your problem as well. This is an old two prong outlet that is NOT as safe as today's three prong ones because they are not grounded. My husband did it quick in an old house we lived in. I would just google "how to replace a two prong outlet." It's a really inexpensive upgrade and I'm sure there are lots of tutorials out there to help you comfortably diy it. It's really not hard.
Looks like you'll have to replace the outlet with a modern two outlet receptacle and cover. Make sure you have power off. There's only two wires on the existing outlet. White goes to silver screw and black goes to brass screw. Yous does not have a ground so you will need a short length of green wire. Connect it to the green screw on the new outlet and screw it to the back of the box.
