How do you decorate a 12 foot long mantle?
The mantle is as long as the room is wide..12 feet , and the fireplace is from floor to ceiling.

use old books to add height to an item, use candle holders of different sizes and colors, look on pinterest.com for some ideas as well
That's very personal and kind of depends on your style or at least the style you are trying to achieve. Pinterest is a great start but this site has many looks as well. You see without a picture or more details it's hard for us to at least get a glimpse of the entire room or to hear from you what you want other than to decorate it. I might say topiary on either side and that's it, an other might offer up skinny tall lamps on either side with a mirror on wall. Or as the previous person offered books and different levels. If you can expand your question it would help us help you.
I have a large picture of my kids framed in the center and just leaning up on the mantle. To the right I have an 'old bike' set of book ends with old books. On the right I have a tall binding of grass standing up and a stand with three small lanterns. Just FYI! the key is a variety and make sure at least one thing has some height to it and one other item takes up a decent amount of space. Here are some more pics:

Start by finding things you love to use as the focal point. Then arrange them on the mantle so that they are asymmetrical ("Not Balanced") - which is more interesting visually. If you are using a group of like objects, use an odd number for the same reason - so they cannot be arranged equally on both sides. Try to find some common ground to other items in the room so the mantle arrangement looks coordinated - by theme, or color or other. But have fun with it - it's only stuff.
This is a great question. Mantels are usually a focal point in a room and can really show off the personality of the owner. You want to keep in mind that it will need items in varying sizes and shapes and in a theme. But mostly you can showcase some of your favorite things.
